Patient Financial Policy

Our practice accepts most major insurance companies.  We strive to provide fast and accurate billing of your office visit as a courtesy to you.  We need your help to bill your insurance.  Please provide our office with complete insurance and demographic information.

All patients must complete registration process and our office staff will assist you in making sure your information is gathered prior to your appointment.  It is your responsibility to notify us immediately if your insurance plan or demographic information changes.  Your insurance coverage is a contract between you and your insurance carrier. While our staff will make every effort to assist you, knowing how much your deductible and co-pays are is your responsibility.

If your insurance requires you to see specific in-network providers, please contact your insurance to see if they will cover services performed by our providers. All insurance policies are different.
